Unplanned Transfer (UPT)

Unplanned Transfer is defined as a combination of 3 adverse events:

1. Return To Hospital (RTH)
   • Excluded from RTH is anyone at hospice at the day of transfer, ED visit at the hospital and planned transfers
2. Unplanned ED visit
3. Death on premise

In other words, a UPT is any emergency where a patient was whisked out, or was found too late to be whisked out.
